Mr. Meinzen would like to post student work samples on the internet with the intent that the students work would be used as an educational tool to be shown to future and current students as well as other educators. Due to the nature of the internet, such work would also be viewable/browsable by the general public visiting such a website.

In order to display student's school work on Mr. Meinzen's website and the internet, written permission is needed from both the student and the parent or legal guardian.

If you are willing to have your child's work placed on the internet along with his/her name as well as used for presentations to other educators, please sign below and return to Mr. Meinzen. If this paper is not returned or signed, then Mr. Meinzen will NOT post your student's school work on the internet.
