A Complete Registration Includes

1. Student Information Sheet [required for all students]
   
2. social security number [recommended]
3. at least two (2) emergency telephone numbers in metropolitan bi-state area
4. signed by legal guardian
5. names of doctor and dentist [should be provided within six (6) weeks]
6. all information complete
7. Student Health Information Sheet [required for all students]
8. Hold Harmless Agreement
9. Medical Release Form
10. Physical Form [required for students in grades K, 6, 9]
11. Acceptable Use Agreement [required if student is to have Internet access; not distributed at registration for elementary]
12. Photographic Release [required if photos of student are to be used; not distributed at registration for elementary]

Proof of Legal Custody and Residency

1. Parent/Guardian authorization form & affidavit of residence [required]
2. Assignment of responsibilities and contact & affidavit of legally responsible adult(s)
   [for parents assigning legal responsibility to non-custodial adult]
3. Current Residents - one or more of the following are needed to prove residency
   1. Driver's license #
   2. Utility bill(s) in the name of the parent/guardian
   3. Voter registration card
   4. Property tax receipt parcel ID
   5. Lease showing the name, address and occupancy date
   6. Occupancy permit or other local government document establishing the parent/guardian as a resident at the stated address
4. Non-Residents
   1. Check for one semester tuition (prorated refund from date residency is proven before end of term)
