The Four Steps in the Student Assistance Program

Step 1

Concerned individuals make referrals to the core team. Referral forms are completed and submitted to the SAP Team. Once a referral is made, the team moves to Step 2 where a case manager is assigned.

Step 2

Information is collected through the use of teacher and school personnel checklists. Only observable behaviors are used to form a picture of the student’s functioning. This includes attendance, behavior, health, and academic performance.

The data is compiled and reviewed by the whole team. A determination is made to either:

      1. Take no action at this time, in dictating that the overall picture of the referred student revealed no significant problems or that the problems are already being addressed by other school or outside resources.
      2. Move on to Step 3, Intervention.

Step 3

The intervention process begins when a problem is identified. The team determines the intervention goals and plans the strategies.

Step 4

The team collaborates with school personnel to monitor the student’s school progress.
